Questions asked in Seasons of a Man's Life

Is there way I can find out the questions Daniel Levinson used when he was interviewing men for the various topcis discussed in the book, Seasons of a Man's life. I am doing research for some professors I work for and they would like to know how the authors came up with and what the questions were that were used in the book.

I passed this question on to Winston Gooden, who did a dissertation under Levinson. Here is his answer:

Dan developed what he called a life structure interview which he used as a guide. Your friend may want to check the Psychiatry department at Yale to see if Dan's papers are in its archives. Judy Levinson, Dan's second wife may also have these documents but I do not know how to contact her. I suppose she might still be in New Haven. With a little work, her contact information could be found.
