OTIS DBR Transistor Burn

We have an OTIS 212MVF and it burn the transistor Darlington of the Brake, regenerative resistances, I changed it and it worked ok for 8 days, and today it burned again, I place a new one and it burned rigth away. I checked the resistors, the suppresors and the diode that has to do with the circuit and everything seems ok.

Check the braking resistor and wiring for a ground fault.

for a 15kw 90 A max 28.4 ohms + - 10 percent for a 22kw 48 to 120A max 18.0 ohms + - 10 percent for a 32kw 72 to 180A max 13.0 ohms + - 10 percent between P and DBR of the rack ovf30
