D76 developer

I have been developing film for a newspaper for the last five years - and have never had any damaged until this past weekend. The film came out of the canister overdeveloped. The photographer thinks she thought she shot the film at 3200 and perhaps actually didn't causing the 25 minute developing time she told me to do was too long. The only other thing I can think is that the canister lid had a crack and leaked out some developer during the 25 minute processing time. Could air have gotten in the canister and if so caused the film to overprocess?
