Don't know if you are still looking for these books, but... Michael Stanbourough sells a number of books on these topics, and I believe you can usually buy them online.

Unfortunately his site is currently in the middle of a transition so I'm not sure when the books will be back up there, but you can always email or phone. The website address is http://www.myo-fascial.com.
