I am writing a contract for labor support services and am wondering what information other doulas include in their contracts.

I include info about what I do/don't do, when I like to be contacted, what my fees are and how they are to be paid, when they would receive a refund, and info about my back-up. DONA has a good contract that I just tweeked a little.

A great book to look into is: Labor Support Forms: A Guide to Doula Charting by Grant. You can find it at: http://www.childbirth.org/CEP.html It is full of sample forms that they allow you to copy and change to fit your needs.

Hi. I know it has been a while since you have posted, but if you are like me then you are always interested in new information and comparing different forms. I have a copy of an excellent contract on a database that I changed to fit my own needs. It is not like any others that I have come across yet.
