The National Transportation Safety Board has somehow "missed" at least 15 aircraft incidents since December 31, 1999, in recording (and perhaps investigating) problems with aircraft. Though it could be that the FAA has taken lead in investigating major incidents, their Incident Data System seems impossible to gather any data from.

10 of these incidents affected a major commercial airliner. 1 accident was fatal.

The data is reported in the NTSB's Accident Synopses, but may not be for accidents/incidents over which the NTSB does not have primary investigation responsibility.
