When fail-proof systems fail

Many folks relate Y2K to the Titanic sinking, the "unsinkable" ship that went down on its maiden voyage. A more recent technological failure cost many innocent people their lives. I'm referring to the Amtrac train that derailed after striking a semi-trailer truck near Bourbonais, Illinois. It was a newer model with rupture-proof fuel cells. The NTSB is currently investigating why the fuel cells ruptured, causing the fire which killed many people trapped inside the wreckage. How could it happen? Remember those words. Perhaps future generations will look back and ask the same question.
